Tue 05-21-13 Pedestrian Struck On the Potomac River Bridge Brunswick Volunteer Ambulance and Rescue Company Inc. - Brunswick, MD  On may 21, around 2300hrs units from 19 and 20 were dispatched to washington county and the potomac
river bridge for a pedestrian struck by a tractor trailer. A197(2V) and RS20 responded on the run.
First arriving units from Washington County went on the scene with a patient struck by a Tractor
Trailer laying on the jersey wall. Unresponsive. A197 arrived and assisted with patient care along
with M1820(WC). Command was established and requesting Aviation for a P1 Category A patient. TRPR 3

Tue 05-21-13 WSF UC Union Twp. Shamokin Dam Fire Company - Shamokin Dam, PA  Today just before 0900 Engine 9-2 was alerted to Union County, Union Twp for a dwelling fire.
Engine 9-2 (Capt. Hoover) were out quickly with 4. Engine 9-2 initially dispatched for RIT was
quickly reassigned upon arrival to do vertical ventilation and then assisted with overhaul.

Tue 05-21-13 Animal in a well at residence Northeast Nelson Fire Department - Bloomfield, KY  Northeast Nelson Fire and Rescue was called out to the report of a puppy in a well. Upon arrival we
found a bell cistern with a small puppy inside that had been there for approx. 3 hours. Mutual aid
was called to Bardstown Fire Dept. where both agencies worked together to extract the puppy and

Tue 05-21-13 Landing Zone For the 2-30 Box Junction Fire Company - Lewistown, PA  Today at 1209 hrs, Rescue 15 was dispatched to assist Companies 2(East Derry), 14(City Hook and
Ladder), 5(Decatur), and 12(FAME EMS) with a rescue assignment for a male patient pinned under a
mobile home. Rescue 15 responded on dispatch with a manpower of 7. While en route, the Rescue crew
was diverted to the Lewistown Armory to set up a landing zone for Life Flight. Crews set up a
100' x 100' perimeter for landing, and relayed the coordinates, hazards, and conditions to
the crew of Life Flight. The Helicopter landed with out incident and its crew began working on the

Mon 05-20-13 VEHICLE FIRE IN THE VILLAGE FREELAND FIRE DEPARTMENT - FREELAND, PA  During the evening Luzerne County started receiving multiple calls for a SUV on fire in close
proximity a four unit apartment building in the Freeland Village. County upgraded the box from just
FFD units to a full commercial alarm due to on scene reports of the exposure becoming endangered.
Chief Joseph J. Stepansky arrived on scene and with his size up determined the exposures were

Mon 05-20-13 Structure Fire Donegal TWP. Claysville Volunteer Fire Department Inc. - Claysville, PA  Station 31 and Station 39 were dispatched for a structure fire in an vacant house at 20:33 hours.
upon arrival of Engine 39 crews faced two mobile homes and an abandon house fully involved. all
structures were vacant. Departments on scene were West Alexander (39), Claysville (31) and Valley

Mon 05-20-13 MVA assist Station 604 Pompey Hill Rd. Friedens Vol. Fire Company - Friedens, PA  At 1207 hrs Station 603 was dispatched with Station 604 for an MVA, one vehicle, with unknown
injuries at the intersection of Rt. 281 and Pompey Hill Rd. Engine 603-2(Shaffer) responded par 4
shortly after dispatch. Engine 2 arrived on scene at the same time as Attack 604-2 confirming
one vehicle on its side off the roadway into a small creek. Crews made patient contact which
was a refusal, provided traffic control, and roadway cleanup. Units were on scene for about an hour

Mon 05-20-13 Tractor Trailer Fire 34.9 SR222N 20130516 West Earl Fire Company - Brownstown, PA  May 16, 2013 at 2222 hours West Earl Fire Company (ST-29) and mutual aid units were dispatched
to MM34.9 SR222 north bound (Warwick Twp) for a reported tractor trailer fire. Responding
chiefs were advised of a fire in a loaded tractor trailer. Deputy 29 arrived, assumed command and
confirmed a working tractor fire with extension into the trailer. Arriving units were able to
extinguish the fire keeping the fire from spreading to the entire trailer. The load was determined
to be butter. Hazmat crews were requested to pump the saddle tanks and assist with runoff. Incident

Mon 05-20-13 Vehicle Crash (Rollover) Call # 13-193 Moon Run Volunteer Fire Company - Pittsburgh, PA  Sunday May 19, 2013 at 21:46hrs Station 245 was alerted for a MVC, 1 tractor trailer rolled over on
I79 North in the area of the S-Bends. 245 Assistant on scene reporting 1 tractor trailer on its
side, victim self extricated denying any injuries. Truck was hauling a turkey bacon
product. 245 Rescue responded with 5 for hazard mitigation. Scene turned over to PSP.

Sun 05-19-13 Fire Department Assists with Body Recovery from Empire Quarry Marshall Township Volunteer Fire / Rescue - Bedford, IN  (NEEDMORE) - Emergency Personnel were called to Empire Quarry in Needmore early Sunday morning after
a report of someone falling into the quarry.
According to Conservation Officer Jim Hash, officers recovered the body of 37-year-old
Jamie Fleenor around 5:30 a.m.
Fleenor, of Bedford, was seen walking a trail at the quarry next to an 150-foot-high cliff, when she
lost her footing and slipped over the edge.
Her husband, David Fleenor, went to a home in the 1000 block of Kentucky Hollow Road and knocked on

dispatched for a Second structure fire at approximately 12:33 a.m.. The second structure was
at the Chalybeate Springs Ministries building along Chalybeate road in Bedford Twp. 31 Chief
advised communications to dispatch Co. 40, dump the remaining apparatus and standby crew in Co. 32

Sun 05-19-13 Structure Fire 41-1 box /Penn St Friendship Fire Company of Bressler - Bressler, PA  Today at 13:29 a reported working fire with multiple calls was reported on Penn St.. Chief 41 arrive
and had fire venting from side A & B with all occupants comfirmed out. E41 crew strecthed an
attackline and went to work, Knocking down the fire in minutes. R49 laid supply line to E41. T50
staged behind E41 and threw ground ladders and set up a PPV. All other apparatus stage and
sent mampower to the front of E41. Investigaters determined it to be accidental.

morning. The bus went off the left hand side of the road into a tree filled median. Two

Sat 05-18-13 Vehicle Fire - Fleming Rd. @ Ekastown Rd. Sarver Volunteer Fire Company - Sarver, PA  Right before 17:00 Saturday afternoon, Stations 36, 27, and 45 Ambulance were dispatched to the
intersection of Fleming Rd. and Ekastown Rd. for a vehicle on fire. Engine 27 arrived first to
find a fully involved Ford Taurus with the occupants out safely. The crew from Engine 27 made
a strong attack with the trash line while Engine 36 arrived and assisted with extinguishment and
